The principal applicant, their spouse or common-law parner...must complete thire own copy of this form.

But on the checklist, this form isn't being included in the spouse section...

Does my spouse needs to provide this form anyway?

For "spousal sponsorship" ONLY the applicant needs to complete and submit one. There are other types of sponsorship, say 'parental' for eg. where both 'parents' (as applicants) would be required to complete and submit their own copy of the form.
