Pages 2-7 there is a space at top of page says "applicants name,date of birth",I put my wife name there as she is applicant?Or for this for am I the applicant?

I believe in that spot you put the sponsors name, since it is a form for the sponsor. The use of the word 'applicant' is a little confusing because you are not the principal applicant, but you are technically the applicant for that form. That box is just to keep track of the pages if they get separated they can put it back together again.

It's better to fill the forms in on a computer via the PDF file. For example in this case, when you fill out Box 1 under "Sponsor Personal Details" it will fill in those boxes on top of pages 2-7.
