IM008 question about Latin alphabet & UCI

Hello, My wife has this letter in her name "Ę". In IM008 it says to input the name exactly as shown in the passport. Should I always be putting in the exact spelling of her name or just in this case, or never? (substituting it with a regualr "E")

Also, what should i be putting in the box that says "UCI"? (question 3 under personal details)

UPDATE: It does not even let me input the letter "Ę". I suppose i should then just use the appropriate latin substitute, being "E"?

That leaves me with the UCI number, can someone explain me what that is?
