IM0008 Question 11 "Last entry date to canada" greyed out

I visited canada this september on a tourist visa. I'm currently a resident of US (H4) and citizen of India. I want to enter the date of my entry in question 11 of IM0008 form. But this field is greyed out to me along with the date field. ?!! why is that? should I write in this column using pen after I print out the doc?

No, don't write anything. It's just misunderstanding. They implied different thing here which is not applicable to your situation. It's something for those who already stay in Canada on a work permit or something, so leave this field grey.
