Identity and Civil Status Documents

I'm sponsoring my dad in Indonesia from Canada and when applying for his PR, we need to send civil documents.

Does anyone know of any examples?

He has a birth certificate and has a name change paper.

He was never legally married and have children from 2 girlfriends.
He never owns a house so no household registrations.

I guess they mean his ID, passport and birth certificate and name change paper. maybe birth certificates of his daughters too.
