Do not sign it unless and until you are told to do so. Neither do you 'submit' it.

You need to provide more information. Where you got it, in what circumstances, etc.

Easiest would be to read ALL of the instructions etc that you received with it.

You need to physically land in Canada. And do not sign it at home!!
