I received my MR and RPRF, but in addition I got an email asking:
> Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. There is a gap in your schedule A. Please answer and sign a newly completed Personal History (question 8).
Stupidly I forgot to give additional info for my Personal History, I had already filled in the five lines and should have given an extra sheet.
Do you think I should give it to the date of my application or to current date? I applied in July but I switched jobs in October. I figure it'd be best to include my most recent job, right?
