How to order GCMS notes?

I am filling the IMM 5563.
Unders section C, which one should I choose?
4 A Status of an application
4 C electronic files - immigration file: permanent residence

My husband chose "status of application" is it enough to understand the reason why our application status under QSW is "application received" after almost 11 months (with medicals expiring in 4 months)?

Please confirm it !

It would have been better if he had ticked 4(C) Electronic files but 4(A) is also good as it will reveal the same info. Thanks
