how to apply for re-entry visa

Hi all, I have been reading the forum and I have checked the form IMM 5556. My wife will need a re-entry VISA and she wants to apply form here in Canada prior to going to the Philippines. Now, can somebody tell me where on the IMM 5556 Form, the specific section that asks to re-enter into Canada? I don't see it anywhere on the form. I see the check boxes to answer A, B, C, d or E listiing what the person is applying for, but again, I do not read anywhere listing re-entry VISA. Section D subsection 10 (My Request) is the only place that remotely resembles a place to request entry back into Canada. Is this the place on teh application where she would submit dates to go to and come back from the Philippines. Thanks.

There are new forms available on the CIC website. I recently applied for a TRV 're-entry' visa - there is no specific re-entry visa, it is still a TRV. May just have been termed as a re-entry visa for those already in Canada and need a visa to re-enter when they return back. More details here:

The latest forms I used are IMM5257 and those included in the application package.
