The sample letters I have seen only list a single salary for the whole duration of each job position held.

However, in my country it is customary for someone's salary to go up every so often even if their position and responsibilities remain the same.

How should we represent salary raises in the employer reference letter?

For the reference letters, I just asked the HR people to quote my salary at the time of my departure from the company.

For showing the rise in salary, I provided paystubs/salary slips to show the same.

I (my boss) stated my finally salary in the letter. I submitted original offer letter with initial salary. I also submitted raise letter - every time I received a raise (I received a letter) n pay stubs as well.
 
Seriously, CIC do not really care about salary increase in the same position. All they want to see is that you were paid for the job (and that the pay was more or less reasonable).
 
Agree. Just report the last salary in the job before moving on thats it.
