How do I upload colour of the eyes and height information request?

I was asked to supply information about my eye colour and height. What is the way to go about this? do I need to go back to the hospital and get a certified document?

Just fill the requested information in a word document with all your personal details like Name,Date of birth,passport number,UCI and file number so that it will be easy for them to identify.Make it into a pdf and upload in your EE profile,as there must be new placeholder to upload the requested information.

No need to go to a hospital,you can get the info from a nearby clinic.
