How do I make a BOWP Paper Application?

I have just spent a while googling and trying to find this but it always leads me to online application.

How do I send a paper application in the mail?!

What payment do they accept and where do I mail it to?

It's not hard at all.

I googled "cic bowp application".

The first search result is http://www.cic.gc.ca/english/work/permit/extend/permanent.asp

Once there, click on "applied for an open work permit" and follow the instructions.
