Help regarding CEC application - Generic Applicatoin form for Canada [IMM 0008]

Hi All,

My work permit is going to expire by 30th May 2014.

What should I put in "To date" field. Currently I mentioned as 30th May 2014 and "From date" is when I entered Canada.

Also regarding place of entry - should I put Toronto or do I need to include any address (initial stay etc).

In the "to" field, you can enter the date you submit your application, or the date your work permit expires.
Both solutions have been used before and were successful.
The place of entry is where you entered Canada, not where you stayed at. So put it the airport, or border crossing where you entered Canada.

"Toronto" what? You fell out of the sky and crashed into Toronto? Or did you take a plane and landed
at the Toronto airport?
If entered Canada at the airport in Toronto, then put "Toronto Airport" in the field. If it was a land crossing,
put the name of the border crossing in the field.
