Did they send you Medical Form, if yes then it's doctor (approved by CIC) responsibility to provide them the required information and if no medical form, email them ask them from which doctor they accept weather from CIC approved doctor in London or any other ordinary doctor. Then get the the medical certificate from doctor and send them.
