HEC only attests your latest degrees but you will have to submit all your educational documents from Matric to Intermediate. Only your latest degree will be attested i.e your undergraduate degree plus transcript. For more information see the link below:

just remember to keep photo-states of all the original documents you are submitting. You will only need to bring the photostate of your passport, original NIC and copy plus the IMM1017 form that you will receive from CIC when medical request is send to you.

If your submitting through VAC then all new and old passports will be submitted. No original are required for medical test except your NIC offcourse.
