Help needed in filling the Netherlands VOG form

I need a Dutch police certificate and i want to know what info i need to fill out in the section about the organisation that request the VOG and what document do i need to send ?

Hi,

I requested this document when I applied for TR2PR. On the form, I just put Immigration, Refugees and Citizenship Canada under section 2.1 name of the organisation .
I printed a PDF of "Guide 5069 – Temporary public policy:Temporary Resident to Permanent Resident Pathway (TR to PR Pathway)" this website and attached it in the application email.
In the email, I said: I'm writing this email to apply for a certificate of conduct. I'm preparing to apply for permanent resident in Canada which requires me to provide this document (Page14).
Files I attached:
1.Passport
2.Proof of the payment
3.Require for VOG page I mentioned above
4.Application form

I received the VOG one month later. Good luck!
