2) Where the payee or holder for value is a federal, provincial, municipal government or agency, a financial institution, a partnership, a corporation or an incorporated company, the form must be signed by the authorized officer and the name and title must be printed.
3) Where an authorized person signs this form on behalf of the payee or holder for value, the capacity or authority under which it is signed must be described.
They said i need to complete this and wait again for another period of time before they refund it.Can you please help me? I am just confused - number 3... "authority under which it is signed must be described" how should i go about this? number 1... "Your signature must be witnessed by someone who is not a relative." So is it best if I let my friend sign this for me since he is not my relative? thank you so much for the time you spent reading this. I hope you can share your insights on this

As for #3 it applies to you only if someone else is making this claim on your behalf
