Section 11, do we need to put workplace address or actual address where you lived? If someone is unemployed or homemaker which address do they put in that section. What should we put name of contact under same circumstances ?

You put the address where you work and a work contact info (your supervisor). If unemployed or homemaker put your residential address with a friend to back this up or your spouse name as contact.
