Help, app returned for my 16 months old son's citizenship certificate

I have sent the CIT 0001 (10-2014) form last month to Sydney proofs 49 Dorchester but today they only sent back my supporting documents without the application saying "The appropriate kit for proof of Canadian citizenship is available on www.cic.gc.ca/english/information/applications/certif/asp" but the only kit is that CIT 0001 (10-2014) form I sent last month. Anyone had experience applying his son's citizenship inland? Thanks.

CIC has been updating their forms. The question is when did you download the form that you used? The fact you mailed it last month is irrelevant if the form was outdated. The last update was made in Oct, 2014. So, I would re-download the whole package from here http://www.cic.gc.ca/english/information/applications/certif.asp and submit it again. Good luck!
