Help: Affidavit for Translation ( what does mean?)

Hi guys,
I'm preparing my application package and i just noticed this in IRCC website regarding the translation

• an affidavit from the person who completed the translation (if they’re not a certified translator);

I've translated my Police certificate ( issued in Poland) to English by a sworn translation.

It looks IRCC requiers to joun an affidavit for all the document translated outside Canada. How this document looks ? it a separate form that the translator should fill?
In my translation, there is the translator stamp and the following note:

''XXXXX the signatory to this document as competent legal translator duly licensed and sworn by the minister of justice, do hearby certifie that the enclosed translation is accurate....''

I'm not 100% sure what this means and it is the key to your question. Who actually translated the document?
