Help/Advice Needed!!

Here is our situation, hopefully someone can help us. My husband went to a DMP here in Ontario, and we did not get the Original copy 2 of the IMM1017 form. I called the DMPs office today, and they have sent everything to Ottawa and all they can offer me is a copy of Option A. What do we do now??? If I get the Option A and a letter from the DMPs office to back this up, or can we contact Ottawa for the proper form that we need. We were told that all we needed was a copy of the receipt and not having this forum or read through the checklist before the appointment as we were on a tight schedule to get the medical done.

Please if someone can offer some help that would be great!! If not I am gonna try to call CIC tomorrow but I know how much help they can be reading from a book!!
