Our daughter is born in US, we got our PR on July 2010 and planning to move to Canada in the month on Feb 2013, my question is

a) Is medicals covered for the visitors (US Citizen) ? if no, medical insurance for her until she gets her PR ?? which is the good insurance company to go with, in terms of claims/ coverage & premium ? she got all her vaccination up to date, do we need to have her full health checkup before travelling

b) Do ourselves need to have recent medical records before entering, how and where to register to avail the health benefits available after we enter, we are planning to stay in Toronto

1) No - health care is not covered for visitors. Your daughter will qualify for health care benefits three months after she becomes a PR. You should purchase insurance to cover your daughter until such a time as she qualifies for health care benefits.

2) It sounds like you and your wife have not been living in Canada over the last year. If that's the case, you will qualify for health care benefits once you have returned to Canada and lived in Ontario for three months. You can find out how to register for OHIP here:

You can purchase insurance from companies such as Manulife and Blue Cross.
 
Note that scylla's answer is correct for Ontario, but not necessarily for other provinces.

Health care is provincially administered, and Ontario is the stingiest when it comes to providing coverage - but even they will cover under certain circumstances (e.g., properly coded visitor records and TRPs).

Most of the insurance companies offer very similar coverage for Canadian visitors. The difference is mostly in whether they offer coverage for pre-existing conditions, or not and since your daughter is still very young I doubt you need to worry about that.You can choose the amount of coverage for her, ranging from $25,000 to $200,000 based on what you are most comfortable with and of course the price.
