In my case my name has always been Given Name "aaaa bbbb" and Last Name "cccc".
But in my employees record the name was Given Name "aaaa" and Last Name "bbbb".
I know I should say Yes in this field and add this name aaaa bbbb as the other name. My question is what all document I would have to submit to justify this. I am in United States and I have no idea how notaries work here. Any help or directions will be really appreciated.
