Got passport request. How do I fill in "Appendix A"?

My wife finally got a passport request from the VO today. I am the sponsor and she is the principal applicant.

Along with her passport, she has been asked to submit a form called "Appendix A - Updated Information Chart". The form has spaces for Principal Applicant and also Spouse or Partner. Does she need to fill in my information as well since I'm her spouse?

i'd like to know this as well. some people have told me that the sponsor's info doesn't need to be included whereas others have said that they did fill in the sponsor's info.

I'm also trying to clarify what to put on Appendix A that I received with my Passport Request. I'm the principal applicant, so I know I need to put my information down, and include a copy of my passport. My wife is my sponsor, and is a Canadian citizen. We also have a daughter who is a Canadian citizen, so was not part of my application for permanent residency. It's not clear if I only include people who were part of the application to receive permanent residency, or just all of my family. Thanks for any help!

Only the applicants, and appendix à is just hieght and eye colour, plus tombstone details (passport UCI and dob).
