Hello,
I hope someone can answer my questions here...

I'm a Hungarian citizen. My twin sister lives in Canada / Ottawa / for 10 years, has a perm. res. card. Last year she had surgeries. She doesn't have any relatives or close friends, so I went to Canada / as a visitor /to help her out. Finally I have stayed for 1 year - with an extended stay permit. I left Canada in febr./2015.
Unfortunately she needs my help again - but I'm not sure when I can go back as a visitor: do I have to wait for 6 months after my leaving or am I allowed to return / as a visitor / after 4 months as well?
My other question is: if I want to help her/take care of her in Canada then I just have a chance to go there as a visitor or is there any other way of it? She can't sponsor me as a relative due to her financial issue...

You will have to prepare some sort of evidence to prove that what you are saying is true. Most probably on your attempt to re-entry Canada you will be asked about this and also about how can a brother stay out of his country for so long (work, studies, personal business, assets, etc.). When asked be prepared to have documents that support your claim.

Also prepare to show how are you going to survive during your visit as you are claiming she doesn't have the financial status to hire someone but she has the financial status to cover you? Seems contradictory and your visit can be considered working in Canada as a nurse could be hired for this. Prepare proof of your financial status.
 
Last year 1 of our friends had / financially / responsibility of my staying - so I was covered... I could proof on the airport my sister really had a serious health problem -because I got a paper from the hospital / not only me wanted to go to help her out, but her doctor wanted a relative there, because she was in a coma :( and we are identical twins, so I am the perfect match for her if she needs blood or organ /... we both have bachelor degree of nursing, so I could help her in every ways...
I am hoping our friend still able to help me by his responsibility - if so, then I can go... but I still have my same question: can I turn back after 4 months of my leaving?

You can turn back whenever you want (as soon as the same day that you leave).

When you visit a friend where do you stay more? in your friends house or in yours?.... so if you are "living" in Canada but with a "visitor" status then that doesn't make sense. At the moment even though you are saying you are a visitor, you are "living" in Canada (the reason is irrelevant) so questions will be asked, and the purpose will be challenged.

Prepare to show evidence of everything you claim.

So... our friend had a "real" responsibility of me: I stayed in his house - and of course my sister, too... so, he always knew what I am doing in Canada: not working, just try to help my sister to survive and get stronger condition. My sister lost her live-in job - means: living place, too -, but our friend could offer his house to stay in... it doesn't mean he could take care of her physically, so he was glad to know I am able to go there.
My sister got back her old live-in job, so now she lives in her apartment. But needs to get away from her job for a while / get some treatments/physical therapy /... during that time probably she can stay there, but if not then also has a place to stay in...

My new question is: if our friend will have responsibility of my staying again, then I should stay in his house again / of course only with my sister /? In fact I thought I can stay in an another place with my sister / if she has her own one / meanwhile our friend helps out if I am in need of it... but probably not recommended? :(

That is no argument at all. Unless you are your friends' dependant he doesn't hold any responsibility for you.

Im curious if you went back to Canada?? how did it went with the immigration?
