Gift deed in the US / Format of Gift Deed

I am a resident of Pakistan
I have an account in the US - I am planning to take the POF as a gift deed from my uncle in the US.

1. What legal document would I have to show? (assuming PK's affidavit/bond paper wont work)
2. Because all the transactions are happening in US - would this be legal document have to be of US
3. Where can I get it from?
4. Anyone have a format for the same?

Just Prepare the gift deed on a white paper and notarize it from your bank in US. This is considered as legal document.
