GCMS Notes - Please submit the following document

I am a inland application with 3 dependents (Spouse and 2 kids), while applying GCMS notes do my wife need to provide consent form and is there any other document that i need to provide ?

I am main applicant and providing file number, dont i get all the notes on that file which includes dependent information ?
also do i need to submit any proof like passport or work permit ?

Mine is similar to your case. AOR is March 11. Medical and Bio completed on 25th March. Trying to order GCMS to know the status.
I don't know what was completed on my case.

I don't think you need to attach anything while applying GCMS Notes. It will include everything about your application.

by the way, how do you know that criminality and info sharing steps completed?

If your dependents do not complete the form, then your notes will omit the info relating to them. Also, no passport or permit is required.

In order to facilitate the processing of future ATIP requests, although not mandatory, it would be appreciated if you provide a consent from all applicants (including dependants over the age of 16, sponsor, co-signer etc) which were included in the file(s) for which you are submitting the request. This will also increase the amount of information which will be disclosed to you.
Please note that if you wish to receive information about a minor who’s information is located within the requested file, you will need to provide us with a consent signed by both parents. Furthermore, in circumstances where a parent has sole custody, a legal custody document will also need to be provided.
