GCMS notes - How to obtain if UCI is unknown?

Hi everyone,

How can one file for obtaining GCMS notes for a file that is 15 years old and that the applicant doesn't remember UCI #. He doesn't even remember how his name was spelled on his previous passport but we can at least guess the name but UCI# is not known.
