gcms notes confusion

i just want to know for gcms note on cic do i need to send both forms or just second one where we write our all information.

By "both forms" I assume you mean "both pages". In that case, no, you do not need to send the directions - they just need the page with your information on it. I've done that every time with the new form and never had a problem.
