Gap in dates

Hi,

I have received my PER on 15 October saying my application is in progress.

But, just now I realised that there is a gap in dates IMM 5669E: Schedule A - Background/Declaration (for my spouse)

In Item no 12, it asks for list of addresses for past 10 years, mistakenly, there is a gap in date for my spouse addresses (who is dependent on me).

Though I have my PER, is there any chance my file will get rejected and return? Plz help.

How do I correct my error now?

Mail to your vise office, explain the situation and ask them which steps you should take to fix it.
Probably this gap is not material as per their judgement.

Do I have to send mail to my local visa office or to CIC?

Obviously you should mail to the authority which handles your case at the moment.
Since you've got your PER it'll be visa office.
