Forms and documents for non-accompany spouse

Does anybody provide ME the information about the forms and documents needed to add non accompany spouse in the application.

Also i heard that spouse should also fill out the background declaration and additional family info? Who will sign the form that my spouse fills out because it's written there signature of applicant?

Which forms are you talking about. Are you applying through express entry or paper based application?
