Format / contents of experience certificate !

Can someone share the format / content of the experience certificate. Someone said here that they have to be submitted in original.Please tell about their attestation, has it to be notorised or company stamp is enough.Please do share.

1)Please make sure they are on company letter head. If they do not give then do not worry.You can get it written on simple paper and get it notorised

2)Yes if you are getting experience certificate from company make sure to get it stamped and get business card of your reporting manager.
