Form IMM 1017 medical report

Hi everybody my passport request is finally here but along with it the new medical request arrived... i have a question on the envelope i received a form 1017 with my picture on it and canada embassy seal... iknow i should i take it with me and give it to the doctor but after that i dont know what should i do: send it along with my medical exams and x ray or with my passport to the embassy... thanks for the help and the support during all this time!

my second question is how long does this take?

You take the form to the DMP, have the medical and the DMP will give you a receipt copy. The results will take about 4-6 weeks to reach the processing office. Send your receipt copy with the Passport.
