Guy's need some help with questions.

form IMM0008

question 7 - have you received your cert de selection du quebec? do i put na in

7a,7b,7c ?

question 2, - have you ever used any other names eg nicknames, maiden names allas?

i have my first name changed years ago should i select yes and add my old first name?


question 10 - current country of residence ? i'm in Canada right now leaving in a few weeks back to uk
I'll be in UK when form is sent to processing centre - do i need to add dates?


question 12 - previous countries of residence

do i put canada and dates as i've been in canada for 5 months left then come back in for another 5 months
again do i add dates?

If this is not applicable to you, put a cross on 7a NO and leave 7b and 7c blank

Probably your Birth Certificate would have the old first name in that case. If so, you should select Yes and provide the old first name as well as documentation that you have changed the first name.

Country of Residence is where your long term-permanent residence is. I believe that's UK. If you have not lived anywhere else for 6 consecutive months, just provide your date of birth in the From section and leave the current date blank. (you can manually write "Present" later).

This section is for places you have lived for six months or more (consecutively). Each of your visit to Canada is presumably less than six months, hence you can leave this section blank. However, in q11, you would have to proivde the Date and Place of your last entry to Canada.

example from my birth to 2012? in UK? as I have been in canada since January of last year.

my total amount of time is around 10 months in canada 5 months each as I left the country and flew back in and was given another 6 months

can anyone confirm
 
I would add Canada. It is not going to hurt you, even if strictly speaking you have not been here for more than 6 months at a time. Just add a letter of explanation of your circumstances. I used to live in South Korea, but would leave and reenter every few months to get a new visa. I still listed that I lived in South Korea during that time.

So I should write this as UK even that I have lived in Canada with my wife since wedding last year ..I have signed a lease for condo ?
 
Dear Kevwoodfine

Are you living there permanently do you old hold a permanent residence visa? if yes, put Canada if you are on a visitor visa then put UK,
leasing a condo does not prove that your are resident visitors can lease a condo too.

Are you just visiting the UK for a short period then returning to Canada to live in the condo? If so, then it is just like any Canadian who goes on a vacation - he or she is still living in Canada. So I would write Canada if that is the case. If you are going back to the UK for a longer time, then say you are in the UK. You can attach a letter explaining the circumstances in more detail.
