Lots of good info here, but I can´t find what I am looking for. Hope you can help:
I am a Canadian citizen living in the US and in the process of filling out the CIT 0001 for my US born daughter. She is mentally disabled, so I am also completing the representative form. My questions are in regards to the CIT 0001:
1. Section 7 (parents of applicant): Other names used by parent (name at birth, maiden name, etc.) - I was married twice. Do I need to enter my maiden name as well as both of my married names into the box?
2. Section 7 (parents of applicant): Date and Place of marriage. Both marriages were dissolved - do I need to leave this box blank? My daughter´s father is "Parent 2" but we are not married.
3. Section 8: Since my daughter is disabled , she is unable to sign, so I would sign as a parent/guardian although she is well over 14 years old, correct?
Should my application package include a doctor´s note stating that my daughter is mentally disabled?
