Could you please confirm in background declaration form Question no.8, "Name of the company, Employer, School, Facility, as applicable". in this column should we also give the addresses also where we worked where we lived etc. Please advise.

If i write only the employer where i was working and the school i attended that would be enough or address is compulsory. any body who did not write the address got rejected. Please advise.

As some people said here, too much information is always better than little information. So if you have the addresses, write it down.

1) In form 5669 Question no.12 regarding address where did you lived. Should i mention the address, say i have gone for 6 days for vacation and 4 months for an assignment to foreign country as well or no need i need to mention my permanent addresses only. Please this is an urgent request.

2) In Generic Application form question no. 12 Previous country of residence during past five years. Before i received the work permit i was on the tourist status to canada for one year should i mention this in the form. As the form says " If you lived in any country other than your country of Citizen or your country of residence". I assume i have become residence after getting my work permit. Please advise.

If any body can advise it would be great. Its urgent.
