Form 5562 (Your travels): only for Paris applicants?

Hi all, there is a form, number 5562-YOUR TRAVELS. Now I just read in the instructions, that this has only to be completed for applicants going through the Paris office? Is that right?

For Western European applicants, only those applying through Paris have to complete form 5562.
