Hello really need your suggestions,

I forgot to put my name on the fee payment form i am paying via DD, do you think this will be a problem?

As per the Instructions guide and document checklist no column should be left blank and incomplete forms should not be sent else the application will be returned.

you can however email CIC about the situation and tell them that you will be sending the new form with the complete details.
