We currently have a 17 year old Indonesian foreign exchange student staying with us in the United States while she attends our local high school. We would like to all visit Niagra Fall in April - I have been told we need to complete a Temporary Resident Visa for her which I have and have started to complete - is this the only form we will need? - somewhere I saw it stated to also complete a "Supplementary information form (38KB)" too - is the second form needed? Also, Once we arrive at the boarder - will be need to go somewhere special? or do we just present all documents from our car as we process through the toll like gate? Appreciate the info.

Yes, you just need to fill out the TRV form, just read through the instructions given and fill out whatever it asks for. Should be pretty straightforward and simple, just fill out the forms required as per the instruction guidelines and document checklist.

Also, you just arrive at the border as normal, they will tell you what to do if anything, most likely everything will be done from your car.

I assume you know that the application and passport are mailed to the closest Canadian consulate in the US? Not just presented at the border. http://www.canadainternational.gc.ca/buffalo/assets/pdfs/visas/52_1_eng.pdf
