Hello Folks,

I know a lot of us are curious about how the Medical Requests and Forms look like. So, I thought of sharing the same.

If interested, you can visit my CANADADO "Immigration Experience Blog" on Wordpress. Address is given in my signature. I'm still working on making the blog a little presentable :-)

Let me just say that this is ONLY to help satisfy curiosity and nothing else. Hope its not illegal :-) If it is, do let me know & I'll remove those scans.

Actually copies of the forms are here: http://www.cic.gc.ca/english/resources/publications/dmp-handbook/appendix-02.asp

See, I didn't know that.

In my 2+ years since I started this process, I never got to check or see those on the CIC site. So, maybe it may help some people who have a bigger habit of visiting canadavisa.com than the CIC website.

BTW, the medical info given on the CIC website (updated on March 4th, 2012) was found to be outdated on March 5th, by moi, in person.

So, Its All Good!

Later.
