Fmaily info form: Mom is not main applicant. Does she need to fill out this form

I am filling out family visit for my dad and mom. My dad is applicant. i wonder whether my mom need to fill out her family info.

If you are inviting them both then they need to fill up the forms separately.

The title of your thread and the start of your sentence is a little bit confusing.

So if you are just applying for your dad (main applicant) then he should be the one filling up the family information form.
