Hello,

Can someone help me out with these questions:

Do fingerprint cards have to be stamped by someone at the police station before mailing out to FBI?
My local police station (NJ, USA) did not have fingerprint cards, as such, I had to print the one's from the FBI website. The detective that took my fingerprints signed and dated the cards. Will this suffice?

Also, what is the purpose of providing photos to the doctor when getting the medical examination? Should I be the only one in the photos or does my husband have to be included as well? Does the doctor keep the photos?

I only got a signature from the officer on my cards (I had already put the date when I filled the card out).

The photos for the DMP should be passport type photos (not sure what the dimension requirements were though, so look through your guide) and should only include the person being examined. I think, though I don't know for sure, that the purpose of the pictures is for visual confirmation that the person examined is the person applying. One of the photos will be affixed (and stamped) to the receipt you get from the DMP which you will include with your application. I am not sure whether all the rest get mailed by the doctor with your file to the Regional Center or if the doctor also keeps a photo on file.
