Filling out FBI Fingerprint Card FD-258

Hello All,

I had a doubt regarding filling out the fingerprint card. The FBI site mentions that only name and DOB are mandatory. Do we need to fill out the other fields like eye color, hair color, race etc? Given that this information is already available on the application form, I was wondering if we need to repeat it on the FD-258 as well.

People who've already been through the process can you please comment?

Just fill everything you know. Its a simple form anyway.
 
I have a few questions.
First as mentioned in the Canadian website in regards to USA police certificate that we have to post all the documents and wait for weeks to get the result.
Same mentioned website offers to submit application online and submit the Finger print card via mail and that will take 5-7 business days to process. Is it ok if i choose to submit my application online and then send my fingerprints along with the confirmation email to FBI ?

Secondly in regards to the fd-258 form , what do i write in following
Employer and address :
Signature of official taking FP: if i do myself what to fill in this ?
OCA no. :
FBI No.
MNU :
MNU armed forces:
ORI:
Please i would really appreciate your help in this regards

Applying online through the new eDO system is the preferred method. You submit the application details and complete the payment online. Once you get the confirmation email, you take a print out of that and send the fingerprint cards along with the print out.

You dont need to fill everything. Just fill out all the info you know to the best of your knowledge. You can put your current employer name and address. Your SSN if you have one. The other fields might not be applicable to everyone. In the fingerprint card that I sent I hadnt filled out every field, except for signing it, SSN, DOB and complete name, the rest were blank and it was accepted. So dont worry too much about it.
