I am filling in the IMM1017 form, it states that you must put a contract address of a person within Canada. However, I live with my spouse in a different country. Does anyone know what to do in this case?

Also when sending the medical form with the application, do you only need to send the IMM1017? Does the doctor complete it on the same day as the medical and hands it back and then it with the application?

I don't see anywhere in Appendix C where it asks for a contact address in Canada. Which region specific forms are you using?

When your immigration medical exam is completed, the DMP will give you a form to send with your application. Sometimes it's an IMM1017, other DMPs just issue a receipt for fees paid for the exam. The DMP send the results of the exam to the Regional Medial Centre that represents the visa office that will process your file - all you really have to send with your application is proof, from the DMP, that the exam was completed.
 
I have a copy of my 1017 here - it's not something I filled out ahead of time, the DMP had it at his office for me.
And mine says nothing of an address for in Canada.


The DMP will complete the form and give you a copy which you send with your application. He keeps one (maybe 2 I can't remember how many copies there are of it) and sends it with your results which are then requested by the IO when your file is processing. You wont' know results of it unless something is found health wise that they'd need to inform you as a DR. Because they need to take blood tests, and some take more than a day, ect.... it will vary on when the DMP mails it I'm guessing. Mine said it'd be 3 days, overnighted to Ottawa.
