Hi,

Starting this thread for us Filipinas, I am just really new here so I hope I could get few of you who have gone through the application process successfully or who have already started early this year.

Could somebody give me a light on how to fill out these forms?

1. I just got married a month ago, can I now use my married last name for application?
How about getting an NBI? Am I gonna use my married name for that?

2. General application form #3 , Am I gonna put the total number of my family members? or ako lang since I am the only one being sponsored by my spouse?

Nalilito. :P

Thank you !:-)

1. Yes! You may or you may not po... sa case ko po, ginamit ko maiden name ko kc gusto na namin I start yung application..Doon ko na Lang I change name sa Canada. 2. Sa nbi ilalagay mo talaga maiden mo kc criminal records yung Chinecheck ditto sa pinas. Meron nman question sa ni if married then ilalagay mo yung surname ng hubby mo. 3. Yung info mo Lang ilalagay mo.

Hi BeeLa will try to answer your questions

1. I think it's better to your your name while you were single if you want to process your docs right away coz it will take time to change your name to your married name coz you don't have record yet ... and your passport and birth certificate are the docs you need to submit... so to avoid confussion, its better to use what is in your ID.

2. Info mo lang ilalagay mo kase ikaw lang iisponsorin nya.

Hope these helps.

Salamat Sis! Anjan ka na pala! Ang bilis... What I did is I change my passport since it will expire in a year- And now- I am using my married name. So hopefully everything will go smoothly... Sana mabilis din!!!

Hi :-) It seems you have been here for awhile, well, I have been reading the forum now and I am trying to be aware of the process and terms. I have all the terms now except COPR- do you know what is this? Thanks!!!
