Does anyone have any experience in using this? We're using it to pay our $1040.00 at our bank.

There were no instructions sent with this form. At the bottom it says "Send to the National Bank of Canada" once payment has been made. IDK if I'm doing that or the bank is. And I don't see where to indicate what the fees are for.
 
I'm not sure what form you are talking about, I'm sure banks are different from province to province. We paid our fee's @ CIBC we were given a form from the bank it was in 3 sections. One was our copy, one went in with our application, and the last section the bank kept I dunno if for their use or if they sent it to CIC. but you would think the bank procedures would be about the same. anyways just take the form to the teller and hand it to her, she should know what to do with it... GL!

LOL I actually FLIPPED the form over when I took it into the bank to ask questions....and there's the three parts you're talking about. :P But, we had to order them from CIC and wait 2 weeks to get them in the mail....and you got yours from your bank!?? (same bank as our-btw) !&@^*&^%$*!! LOL

Oiy...this monkey needs to get off my back so I can have a brain again!! :-\

my husband paid online since it seemed more practical. Hurry up girl! so you can file this week :-)

Like you found out, there's three parts. Each one says where it has to go. One stays with the bank, one with you, one to go with the application. Just tell the bank how much. And the information on it can be YOUR information OR your spouses. :) Good luck! Congrats on being so close to sending it off!!

LOL well I WAS ready...now to re-print the UPDATED forms!! BAH! Really, they should only get one update a year. This is crazy-NESS I tell you!!
