In "Schedule A – BackgroundDeclaration [IMM 5669]"

- I'm a Canadian citizen that will be sponsoring my American wife.
> No children
> Not sponsoring any other family members


My first question:
Is this form one we each have to fill out or just the principle applicant?

Second Question:
On #6. 'Have you, or, if you are the principle applicant, any of your family members listed in your application for permanent residence in Canada, ever: ..."

- "Personal details of your Father/Mother are listed for Questions 4. & 5." Is this question asking the principle applicant to check the boxes for 'YES' if their father/mother have "had any serious disease or physical or mental disorder" or do these questions only apply to the principle applicant?

For background/clarification my wife's mother has a mental disorder (she's bipolar) and we want to know if the boxes that they ask you to check are solely for the principle applicant or if they also apply to the applicant's parents. I am not sponsoring my wife's parents.

Thanks that answers my first question. What about the "YES"/"NO" boxes for question #6 on the form? Does "family listed in your application" refer to the applicant's parents that were listed or just the applicant? See below

YOU ARE RIGHT IF FATHER MOTHER ARE NOT ACCOMPANYING, then it does not need to be filled
 
My so called wife filled NO in all , and was OK
