Family name missing in Educational documents

Good day all,

The name on my educational documents isi missing my family name BUT all other documents like passport, pr card have family name
So what should I do in such case?
I thinking of getting affidavit from my home country which mentions that both names are for same person
Let me know if this is correct or should I do something else
