Family Form for PGWP

Hello everyone,

I am about to submit my PGWP application online and one of the required form is "Family information form".
Why it's not on the document checklist?
Do i have to submit it as well?

If your part time semester was during an academic session(Fall/Winter) , Don't be surised if you've been refused. One of my friends was refused just because he took 2 courses in his first year in his 6 year MD program.
