To Whom It May Concern:

I received my Residence Questionnaire for Canadian Citizenship.
I completed the Residence Questionnaire, and attached some of the
necessary documents, and mailed it over.

However, the Residence Questionnaire asks for the OHIP Health Records
from the Ministry of Health and Long-term care.

I sent out a separate request by postal mail to the Ministry of Health
and Long-term care asking for my health records.

It's been around 3 weeks.

I posted it to:

Ministry of Health and Long-Term Care
Personal Health Information Office
49 Place d'Armes, 4th Floor
Kingston ON K7L 5J3

I included the following details:

•Date
•Return address
•Health card number
•Date of birth
•Time frame of the personal claim history
•Signature

It's been around 3 weeks which is too long.
It's probably all the Government Bureaucracy that deserves blame. Please tell me how I can follow-up, or at the very least file a complaint about government bureaucracy.
What should I do? I need the Personal Health records for the residence questionnaire.

Thanks,
